Required Immunizations

  • On-time vaccination throughout childhood is essential because it helps provide immunity before children are exposed to potentially life-threatening diseases. Vaccines are tested to ensure that they are safe and effective for children to receive at the recommended ages.  It is the responsibility of our school district to ensure that all students up to and including those 18 years of age, either entering or currently in attendance, be properly immunized.

  • Please contact the nurse's office for specific immunization forms and requirements.  

  • A child shall not be admitted to school or allowed to attend classes, in person or virtually, without a doctor's ceritfication of required immunizations.  

  • In June 2019, Governor Cuomo signed groundbreaking legislation eliminating all nonmedical exemptions for childhood vaccinations required for public, private, and parochial school attendance. If your child requires a medical examption please contact district administration to obtain the appropriate paperwork.

  • When certification is complete pleae return the proper form to your child's school.  These forms can also be faxed to individual school.
