  • New York State Education Law prohibits school personnel from administering medication to students in school without proper documentation from your students primary care physician. Please contact the School Nurse for the proper forms to be filled out by the prescribing physician.

  • To administer medications within the school setting, Maryvale Union Free School district has the following policy which you can access by clicking on the following hyperlink.

  • Administration of Medication

  • Please do not send your child to school with any medications, prescription or over the counter medications, including cough drops, eye drops and anti itch creams. If it necessary for a child to receive prescribed or over the counter medications, please ask your doctor to arrange to have your child's medication scheduled, if possible, outside of school hours.

  • When circumstances make it necessary for your child to take medication during school hours, the school nurse will cooperate with you and the family physician.  Please provide school authorities with a written request from the physician which includes the frequency, dosage, name of prescribed medication and why it needs to be administered during school hours. Your physician may fax the orders directly to the Health Office at 631-4858 or 631-7499.  School personnel will administer the required medication during school hours in accordance with these instructions.  These written requests must be renewed each school year.

  • Medication is to be brought to school and turned over to the school nurse by an adult in the original container from the pharmacy which filled the prescription. The medication will be kept in the Health Office.

  • Medication will only be dispensed under the conditions described above.
