Sports Information and Forms

ALL STUDENT ATHLETES WHO HAVE TESTED POSITIVE FOR COVID-19 AT ANY TIME ARE NOW REQUIRED TO HAVE THEIR PHYSICIAN CLEAR THEM TO PARTICIPATE IN SCHOLASTIC SPORTS. PLEASE CONTACT YOUR CHILD'S PHYSICIAN FOR AN APPOINTMENT!!

Per NYS, ALL students who wish to play a scholastic sport MUST have a current physical on file to be eligible to play a school sport.  Please call the School Nurse or Mr. Klimtzak to inquire about the last physical on file for your athlete.  

Fall 2022 Sport Physicals:   

  • Tuesday, August 9, 2022   8:00 am-12:00 pm in HS Nurse's Office

Sports

All students who wish to play an interscholastic sport must have a current physical and no gym/sport restrictions. It is always best for your son/daughter to see their Primary Care Physician for a yearly physical as they know your child best. You can upload your child's physical directly onto FamilyID, or fax (631-7404), email (meyer@maryvaleufsd.org), or it can be brought to the HS. If you cannot provide a physical from your student's PCP, they can be seen by the school physician at a scheduled day and time. 

Maryvale Schools has moved to an online platform for sport sign ups. Please see the link on the Athletics Page for FamilyID or click the link below. No paper frms will be accepted for sport registration. The only way to register for a sport is through FamilyID.

  • Varsity and Junior Varsity sports begin

  • Modified Sports begin

  • Boys Varsity Swimming and Diving

  • Boys Varsity Bowling
