Physicals

New York State Education Law requires physicals be done for all students in Pre-K, Kindergarten, and Grade 1, as well as, any student new to the district. The information contained in a physical is necessary in providing a clearer picture of your child's health needs. This important document should be given to the health office as soon as possible for each year it is required. In order to be acceptable the physical needs to be dated within one year of the start of school and be signed by a physician. Please have your health care provider send a copy of the most recent New York State Physical to the health office. The FAX number is (716) 651-0031.

ALL STUDENTS MUST BE IN COMPLIANCE WITH REQUIRED NY STATE IMMUNIZATIONS. STUDENTS WHO ARE NOT FULLY IMMUNIZED WILL BE EXCLUDED FROM SCHOOL

Immunization Requirements:

Pre- K                                                               

  • DTaP/DTP      4 doses 

  • Polio                  3 doses

  • MMR                 1 dose                                 

  • Hep B                3 doses  

  • Varicella          1 dose

  • Hib                  1-4 doses

  • PCV                 1-4 dose

Kindergarten-Grade 5

  • DTaP        5 doses (or 4 doses if the 4th dose was received at 4 years old)

  • Polio        4 doses (or 3 doses if the 3rd was recieved at 4 ys old or older & 6 mo apart)

  • MMR        2 doses

  • Hep B        3 doses

  • Varicella   2 doses

**DTaP = Diptheria and Tetanus toxoid containing vaccine and Pertussis

MMR = Measles, Mumps, Rubella

 Varicella = Chicken Pox
