Health History/Emergency Information

Health information forms contain important medical history pertaining to your child. Medical conditions, allergies, medications and restrictions should be listed and kept updated since the original form is completed at the time of registration.

Emergency cards are sent home with students on the first day of each school year. It is extremely important to complete the information and return it to school the following day. It requests parental contact information due to illness or emergency. Additional contact persons are requested for times when a parent or guardian cannot be reached. Please notify the office or nurse regarding any change of phone numbers or contact persons during the school year.
